Please complete this form if you are requesting a change of dismissal for your child. Only requests using this form will be considered an official change of dismissal request. DO NOT email or text your child's teacher or any other staff member. Do not send a teacher or staff member a message on Class Dojo. If you have more than one child, you will have an opportunity to add their names at the bottom.
For your child's safety, we WILL NOT take any changes of dismissal over the phone at any time for any reason!! Your submission of this form will serve as your signature and be viewed as an official request.
No changes of dismissal will be accepted after 12 P.M. (noon).
On Early Release days this time will be 10 A.M.
